How does hair drug testing work?

Hair analysis is widely used in forensic toxicology because it allows laboratories to assess patterns of drug exposure over extended periods.

When a person consumes a drug, the substance enters the bloodstream. As hair grows – at an average rate of approximately 1cm per month – small amounts of these substances become incorporated into the hair shaft.

Once embedded, they remain relatively stable. This allows laboratories to analyse a small lock of hair and build a retrospective overview of exposure over weeks or months.

For example, a 3cm hair sample taken close to the scalp will reflect approximately three months of consumption history.

If this sample is segmented into 1cm portions, the analysis can provide a month-by-month overview. For samples longer than 3cm, the detection window may be further extended.

This is one of the unique features of hair when compared to other biological specimens, making it particularly suited to assessing patterns of use rather than very recent consumption.

How do drugs get into hair for drug testing?

Drugs mainly reach the hair follicle through the bloodstream. However, once the hair strand has emerged from the scalp, drugs can also be deposited onto the hair through sweat and sebum, the natural oils produced by the skin.

Additionally, external contamination from the surrounding environment can contribute to the amount of a substance present on the hair.

The concentration ultimately measured by the laboratory therefore reflects several biological mechanisms rather than a single pathway of entry into the hair.

This is one reason why drug concentrations found in the hair should not be interpreted as a direct measure of how much the person has consumed.

When is hair drug testing most reliable in legal and court cases?

Hair drug testing is most reliable when assessing regular or repeated use.

Where a substance is consumed over weeks or months, repeated exposure increases the likelihood of incorporation into the hair, producing a more consistent and detectable pattern.

In practice, hair testing is commonly used to:

  • Evidence long-term drug use
  • Evidence abstinence over a certain period
  • Assess adherence or misuse of prescribed medications
  • Support safeguarding decisions
  • Corroborate or challenge self-reported substance use/abuse histories
  • Provide longer-term context where blood or urine tests are no longer informative

Drug testing using hair does not determine exact daily use or precise dosing. Instead, it helps establish whether use appears to be consistent, intermittent or absent over a defined period.

When can hair testing be used to identify one-time drug use?

There are instances where identifying single use (exposure) can be critically important, as discussed below.

However, one should keep in mind that while high-dose or highly detectable substances may sometimes be identified after a single consumption event, a negative result does not definitively exclude isolated exposure.

1. Hair drug testing in suspected drug-facilitated incidents

In cases of suspected drink spiking or drug-facilitated assault, blood and urine samples may no longer contain detectable levels after a few days. Hair testing, if performed several weeks later, may help identify whether a drug was present during the relevant time frame.

2. Hair drug testing in child protection and safeguarding cases

In safeguarding cases, professionals may need to determine whether a child has been exposed to drugs within the home environment, and whether this reflects a single event or ongoing parental substance use.

While inhalation is the most common route for smoked substances such as cannabis or crack cocaine, children can also be exposed through direct handling of drugs that have been left unattended or accessible in the home.

Hair analysis can help differentiate between exposure and consumption, supporting proportionate decision-making.

3. Hair drug testing in legal disputes, family court cases and professional investigations

Where there are disputes about alleged drug use, for instance in family court proceedings or professional investigations, distinguishing between a one-off incident (e.g. alleged drink spiking, claims of accidental ingestion or single episodes of experimentation, disputed relapses, etc) and sustained use can significantly influence the outcomes.

Here, hair drug testing may provide contextual evidence that supports or challenges allegations, particularly when other biological samples are unavailable.

4. Hair drug testing in clinical scenarios

In medical settings, hair testing may occasionally assist in clarifying whether a medicine was taken once, intermittently, or regularly; this is of relevance where adherence to therapy or misuse is in question.

What are the limitations of hair drug testing for identifying one-time use/exposure?

Despite being possible, detecting a single consumption event is more complex than identifying repeated use.

This is because many factors may affect incorporation into hair, namely:

  • The type of substance
  • The dose consumed
  • Individual metabolism
  • Hair colour and cosmetic treatments

Additionally, the analytical sensitivity of the laboratory’s instrumentation also plays an important role in identifying those events.

Indeed, a low-dose, one-time consumption may fall below detection limits. Therefore, interpretation of hair testing results must always be undertaken by experienced professionals and considered alongside clinical, behavioural or investigative information, as part of a wider evidential framework.

Does hair colour or hair type affect drug test results?

Many drugs bind more easily to melanin, the pigment that gives hair its colour. This means that darker hair may pick up certain drugs more easily than lighter or grey hair.

Cosmetic treatments, including bleaching, colouring and chemical straightening, also have the potential to alter the hair structure and reduce the concentration of some drugs, while sun exposure and physical damage can also influence the amount of drug that remains in the hair shaft.

Consequently, concentration differences between two individuals do not necessarily reflect differences in drug intake alone.

This explains why drug concentrations should not be compared directly between individuals or interpreted as an exact measure of consumption.

In scenarios where timing is critical, combining narrow-window tests (e.g. oral fluid or urine) with wide-window hair analysis can provide a more complete picture.

Hair testing is a robust and well-established method for assessing patterns of drug use over time.

It is most effective for identifying sustained consumption, but in certain circumstances, it may also assist in investigating single consumption events.

Understanding its strengths and limitations ensures that results are interpreted fairly, responsibly and in context, particularly where decisions may have significant legal, clinical or safeguarding consequences.
